1. Introduction
By booking a home sleep test, you agree to comply with these terms. If you do not agree with any part of these terms, please do not proceed.
2. Eligibility
Home sleep test is open to individuals who are 18 years of age or older.
3. Time Period to do the test
Home sleep test must be completed within 6 months from date of booking.
4. How to Participate
Complete the home sleep test booking form and make payment.
5. Home Sleep Test Kit
Participants will receive a home sleep test kit on the date selected in the booking form. The kit must be used strictly in accordance with the provided instructions.
6. Change the Test Date
Participants can change the date based on availability by contacting Smartfuture at screen@oursmartfuture.com or +65 8836 0984.
7. Limitation of Liability
Smartfuture is not responsible for any lost, late, misdirected, or incomplete entries, or for any technical or human error that may occur while conducting the home sleep test or preparing the report. Participants agree to release and hold harmless Smartfuture from any liability arising out of them doing home sleep test.
8. Changes to Terms and Conditions
Smartfuture reserves the right to modify these terms and conditions at any time.
9. Governing Law
These terms and conditions are governed by the laws of Singapore.
By participating in the Campaign, you acknowledge that you have read, understood, and agree to these terms and conditions.
